Denver Indian Health and Family Services, Inc. is Hiring a Family Nurse Practitioner Near Denver, CO

Denver Indian Health and Family Services is recruiting a Family Nurse Practitioner (FNP) with unrestricted Colorado License and full prescriptive authority. DEA certificate is required. Duties include covering a full age spectrum of patients in a small clinic setting. Care provided is primarily by appointment or walk-in, mobile health rotation, and includes chronic care and low acuity, low volume,care. The clinic primarily serves American Indian/Alaskan Natives and large part of the chronic care involves the care of patients with adult onset diabetes.

The Nurse Practitioner works under the general direction of the director of primary care to provide a wide range of primary care services which encompasses health promotion, injury and illness prevention, diagnosis, medication assistant therapy (MAT),) and medication for opioid use disorders (MOUDs), treatment and monitoring of commonly encountered health problems and injuries, monitoring of stable chronic health problems, and community development and advocacy. This position works with the Mobile Health Unit and must be flexible and willing to travel.

Assessment and Treatment of Patients

· Determines and assesses the health status of patients by reviewing records, taking comprehensive medical histories, performing physical examinations, and records findings.

· Assesses and treats acute and chronic illnesses and injuries of patients of all ages.

· Performs wellness assessments of adults 18 and over and children 0-17.

· Works with integrated care team to assess mental health substance use disorders with multivariate evidence-based interventions that include therapy and prescribing MAT/MOUDs.

· Works with patients to determine obstacles and use a structurally informed perspective to health care.

· Initiates, performs, and works with clinical team to perform screening and diagnostic tests, including blood draws, cultures, skin tests, pap smears, and other procedures appropriate for patient’s condition.

· Informs, counsels, and instructs individuals and families on their specific health problems, test results, and general health-related matters such as nutrition, birth control, social behavior, growth and development, aging and substance abuse.

· Recognizes situations requiring physician care, such as acute conditions requiring immediate attention, changes in chronic conditions, indications of early stages of chronic conditions, mental and emotional disturbances, and communicable disease cases; consults with and makes appointments with physicians for patients.

· Return patient phone calls re: follow-up questions, lab results, prescription refills.

· Prescribes medications according to proper clinical care and standards of practice.

· Signs and/or faxes prescription refill forms daily.

· Prepares and sends referrals for patients to specialty clinics and coordinates care.

· Review laboratory results and interpret results.

Patient and Staff Education

· Provides patient education to patients and coordinates the development, revision, and implementation of patient education programs, materials, and resources for DIHFS.

· Identifies education needs of staff and plans, implements, and evaluates formal educational programs to meet staff and agency needs.

· Works with patients who use drugs without judgement / stigma to reduce harm and support recovery.

Documentation and Tracking

· Prepares and maintains complete and accurate charts, reviews material prepared by other staff for completeness and accuracy.

· Record chlamydia results on spreadsheet per State of Colorado Dept. of Health guidelines.

· Track data for other organizations such as Women’s Wellness Connections grant.

Health Clinic Coordination

· Manage and prioritize health clinic tasks, delegating effectively to provide timely and appropriate health care for patients.

· Consults and confers with a wide variety of health and related care providers, such as public health nurses, social workers and mental health workers.

· Establish and monitor standards of nursing care for Clinic by working with individual staff, managers, and other health-care providers.

· Initiate, implement, and evaluate policies and procedures related to patient care.

· Identify problem areas and recommend changes and solutions.

· Study changes in American Indian health and seek additional knowledge on unusual problems and conditions, new treatments, and medications.

· Demonstrate quality nursing care, problem solving skills, effective use of resources, collaboration with other health professionals, and professional standards.

· Plan and implement clinic and interagency cooperation to provide for continuity of patient care.

Actively participates in staff meetings, in-service, outreach, and educational programs.

Flexible in covering clinic as needed.
